Care at Home I/II Program Complaint Process
Do you have a complaint about the Care at Home (CAH) I/II Program?
Complaints on behalf of a child enrolled in the Medicaid CAH I/II waiver about specific waiver services should primarily be made directly to the provider agency.
Complaints about Medicaid waiver service(s) or provider(s) may also be directed to the child's Local Department of Social Services (LDSS) at the number listed below, or directly to New York State Department of Health (NYSDOH) CAH I/II staff at 518‑486‑6562.
NYSDOH also operates a Home Health Care Hotline that affords individuals an opportunity to register grievances or complaints about home care services. To file a complaint, call the toll free hotline at 1-800-628-5972.
NYSDOH waiver management staff will acknowledge by letter complaints they receive. The final determination will be communicated in writing to the person initiating the complaint, as appropriate and allowed by HIPAA confidentiality regulations.
