#20561 Medicaid Redesign Team Health Homes Supportive Housing Program
Pursuant to New York State Finance Law §163.10(b), the New York State Department of (DOH) is presenting the following summary of relevant circumstances and substantial reasons why a competitive procurement was not feasible.
Description: The Office of Health Insurance Programs is amending the 16 Health Homes Supportive Housing Program (HHSHP) contracts. These amendments will ensure there is no gap of service while finalizing awards resulting from the RFA (#20443 Medicaid Redesign Team Health Homes Supportive Housing Program) to reprocure this Program.
The Department seeks to add seven (7) months (10/1/2024-4/30/2025) to the existing 6-year contract period of 10/1/2018-9/30/2024 at various amounts for all 16 of the grants. The proposed contract amendments would result in a new contract end date of 4/30/2025.
