CRER # 20782  Community Health Worker Initiative

Pursuant to New York State Finance Law § 163.10(b), The New York State Department of Health is presenting the following summary of relevant circumstances, and material and substantial reasons why a competitive procurement was not feasible. The New York State Department of Health [Program Name] Program has made a determination to issue a Single Source Procurement to # eligible grantees/vendors to support ongoing currently funded projects.

The New York State Department of Health (NYSDOH), Office of Health Equity and Human Rights (OHEHR) Executive Team, has been approved for a one- year contract extension of the current Community Health Worker initiative in place with Joseph P. Addabbo Family Health Center and Jamaica Hospital Medical Center. OSC previously approved a two-year pilot Community Health Worker Initiative with both entities to address adverse health outcomes and significant health and wellness needs in the pocket communities of Far Rockaway and Jamaica, Queens.

Due to the specific circumstances of this procurement and the unique qualifications of the selected firm/entity, a competitive procurement was not feasible in this instance.
