MAP Plans' Disenrollment If Enrollee Has Been Absent 30 days
The following Medicaid Advantage Plus Plans may initiate disenrollment when the enrollee has been absent from the plan´s service area for more than 30 consecutive days:

- Hamaspik Medicare Choice
- Healthfirst CompleteCare
- Integra Synergy MAP
- MetroPlus Health Ultracare
- RiverSpring MAP
- Senior Whole Health of New York MAP
- VillageCare Max MAP
- VNSNY CHOICE Total
- WellCare Fidelis Dual Plus *
* WellCare Fidelis Dual Plus has now also signed the model contract with the updated involuntary disenrollment reason for members absent 30 days from the plan's service area.
