The purpose of this General Information System (GIS) message is to advise local departments of social services (LDSS) of revisions made to the LDSS-4038, "Explanation of the Excess Income Program".
The Department form number of this document has been changed from LDSS-4038 to OHIP-0026 (11/10). The LDSS-4038 will be removed from the electronic Library of Official Documents (http://health.state.nyenet/revldssforms.htm) and will be replaced with the OHIP-0026.
The explanation was also revised to make it easier to read and understand, and to clarify existing policy. Although revisions have been made, there are no policy changes in the area of excess income.
