The purpose of this posting is to notify all Managed Long Term Care (MLTC) plans operating in the New York City area that the application for the QIVAPP is now available. This application should be used by MLTC plans seeking consideration, on behalf of Personal Care and Home Health Aide providers, to participate in the QIVAPP Program. Applications must be submitted via email to HCWorkerParity@health.state.ny.us by Tuesday, September 2, 2014
