Population Health Registry

As of December 31, 2018, the Population Health Registry stopped accepting registrations and onboarding EPs. The registry continues to collect data from EPs who were already submitting production level data.

The Population Health Registry was available to Eligible Professionals (EPs) inside the five boroughs of New York City.

The Population Health Registry collects information from NYC EPs on Aggregate Influenza–like–Illnesses (ILI) and NYC EPs utilizing eClinicalWorks can join the NYC Macroscope Hub, which is a surveillance system that tracks conditions managed by primary care practices. Some of the conditions include obesity, diabetes, hypertension, and smoking rates.
