Ms. H

Mrs. H, a 79-year-old female, was enrolled to Healthfirst FIDA AbsoluteCare on May 1, 2015. During an Interdisciplinary Team (IDT) meeting with Mrs. H and her son, her son stated that they had no new goals or issues, and that that they "seem to always be ahead of the game."

However, in October 2015, Mrs. H was hospitalized. After returning from her hospitalization, the IDT once again came together to adjust the Person-Centered Service Plan (PCSP) for Mrs. H. An RN and PT were put in place for an evaluation and fall prevention plan. Since then, she has been doing much better. Her mobility and continence has improved, with the result of fewer accidents and a happier, more confident Mrs. H.

Mrs. H.´s son was very satisfied with Healthfirst´s FIDA services and the current PCSP, which was tailored to Mrs. H´s needs, even after they changed. There has been no falls or admissions since the evaluation and fall prevention plan were put into place.
