Ms. P and MetroPlus
Ms. P is an 89-year-old female who is alert and oriented x 3, meaning she is responsive and alert to her environment, knows who she is, and where she is. She enrolled into MetroPlus’ FIDA plan in May of 2015. She lives alone in an apartment building and has Patient Care Services twenty-four hours a day, seven days a week. Her primary decision maker is her daughter, who lives a few blocks away from her. Being wheelchair bound, Ms. P requires assistance with activities of daily living and instrumental activities of daily living. She also requires Hoyer lift transfer from bed to chair.
Ms. P’s current diagnoses include chronic systolic congestive heart failure, dysphagia following cerebral infarction, essential primary hypertension, gastro esophageal reflux disease without esophagitis, major depressive disorder, poly osteoarthritis unspecified, unspecified hearing loss and urinary incontinence.
Recently, Ms. P was placed on oxygen due to congestive heart failure. An Interdisciplinary Team (IDT) meeting was held on Friday, May 13, 2016. During the meeting, Ms. P’s aide expressed that she thought Ms. P was depressed because she was lonely and isolated. She stated that since “being attached to the oxygen tank,” Ms. P is unable to go to her activities or maintain her regular social interaction. The Care Manager, also part of the IDT, suggested that she obtain a portable oxygen tank so that she will be able to go to her social events and see her friends in the building.
The portable oxygen was ordered and delivered a few days later. Since then, Ms. P has been able to attend previous activities and resume socialization with friends. She is now more cheerful and has shown significant improvement, increased communication and appropriate social interaction.
