Childcare, Child with a Disability, Adult Dependent Care and Elder Care Related Reimbursement Verification

To support authorization of care reimbursement for all child, child with a disability, adult dependent, or elder care expenses due to a living organ donation, supporting detail and documentation is required. Failure to submit required documents that support reimbursement risks non-reimbursement or denial of lost care claims.

All documentation is reviewed by the Living Donor Support Program. If further supportive documentation is required for reimbursement the Living Donor Support Program will seek to request further supportive documentation.

Documentation required to prove care provided:

  • New York State-Living Donor Support Program Childcare, Child with a Disability, Adult Dependent Care, and Elder Care reimbursement forms
    • Forms need to include:
      • Name of individual(s) receiving care, date of birth, care provider name, care provider address, care provider telephone number, and care provider email address
      • Date and number of hours the care was provided
      • Attestation signed by the living donor
    • Child with a Disability and Dependent Adult Forms need to include:
      • Documentation of qualifying diagnoses by a physician, licensed, or certified psychologist or other professional with the appropriate credentials to make such diagnosis and documentation
