Local Health Department Contact List for the Health Equity Impact Assessment
The Department encourages Independent Entities to collect feedback from local health departments (LHDs) as part of the Health Equity Impact Assessment. Public health experts offer valuable subject matter expertise and objective input on community and public health matters, which will enhance the overall evaluation of Certificate of Need projects.
To best engage with LHDs, include "NYSDOH Health Equity Impact Assessment" in the subject line of the email to ensure it is not mistaken for spam. The following information must be sent to an LHD prior to meeting: 1) A summary of the Certificate of Need project (including proposed location and services), 2) service areas involved (i.e. zip codes), and 3) a timeline for project implementation.
