Licensed Home Care Services Agency Applications
The Licensed Home Care Services Agency (LHCSA) Certificate of Need (CON) application is now available. Please read through the Dear Administrator Letter and carefully review all of the attachments to become familiarized with the new LHCSA CON requirements. NOTE: Policy and Procedure Manuals must be submitted AFTER the LHCSA application has been approved by the Public Health and Health Planning Council (PHHPC) and Department of Health.

2024 Licensed Home Care Services Agency Statistical Report and 2024 Assisted Living Program Licensed Home Care Services Agency Statistical Report
The 2024 Licensed Home Care Services Agency Statistical Report and the 2024 Assisted Living Program Licensed Home Care Services Agency Statistical Report is now available for completion. Reports must be submitted by December 31, 2025.
