Building a Person-Centered Plan of Care: Supporting Quality Plan Development and Implementation

The NYS Department of Health, in partnership with the Office of Mental Health, is pleased to announce the availability of a new, online training series for Health Home Serving Children Care Managers and their Supervisors on Building a Person-Centered Plan of Care: Supporting Quality Plan Development and Implementation.
This training series, which was developed in response to provider needs and feedback, will build on previous trainings and will help care managers to expand their skill set by connecting key steps in the care planning process, including the CANS-NY, comprehensive assessment, and family engagement, to develop and maintain a person-centered Plan of Care (POC) in collaboration with the child/youth and family.
Specifically, this training series will focus on a person-centered Plan of Care approach to:
- Understanding the care manager's role in developing a Plan of Care that is mutually crafted by the child/youth and family and care manager;
- Engaging the child/youth and family in translating their strengths and needs into a Plan of Care;
- Effectively incorporating required assessments to the creation and maintenance of the Plan of Care;
- Explaining, supporting, and educating the child/youth and family in understanding and participating in the care planning process;
- Discussing approaches for working with the family to generate the family's vision, goals/objectives, and potential services; and
- Working with the child/youth and family on strategies to monitor and update the Plan of Care.
A pre-requisite to register for this POC training series, learners must have completed the CANS-NY General training series, as well as Family Engagement training.
To ensure that care managers and supervisors are able to register for the Person-Centered Plan of Care training, additional Family Engagement trainings have been added to the CANS-NY Institute Training Calendar.

Please note that Person-Centered Plan of Care trainingcourse will eventually become a requirement for all HHSC care managers and supervisors. Additionally, in-person trainings may be offered, when deemed appropriate.
