What to Report
Report any adverse event that threatens the confidentiality, integrity or availability of government resources, or that involves misuse of computing resources or is disruptive to normal system or data processing operations, including:
- Misuse, criminal or unauthorized use of systems
- Data theft or accidental disclosure of data
- Unauthorized access attempts
- Unauthorized viewing and/or access to sensitive data

How to Report
In the event of an incident please fill out and submit the Incident Reporting Form to the Incident Response mailbox. Reference your DUA Identification Number in the email heading.
