Successful implementation efforts will be discussed, as well as an update on the changing regulatory landscape and available resources. Panel discussion will address the data used, the types of care pathways and handoffs required, as well as the public education campaign approaches that have been effective to date. |

A panel discussion will cover establishing a hospital–based telehealth/telemedicine program. Northeast Telehealth Resource Center will introduce the session with a brief presentation on the changing telehealth landscape, reimbursement policies, and emerging telemedicine programs nationally. Community Partners of Western New York will speak about their early successes and overcoming of challenges during the launch of their hospital–based telemedicine program. |

The session will explore in more detail the path towards developing early stage performance–based contracting and how this has resulted in better alignment of project goals between the PPS and CBOs. |

Session A (PDF) Engaging Front–Line Providers |
Persian Terrace | Joel Hornberger, Cherokee Health Systems | Motivating and engaging front–line providers can be challenging, but there are multiple avenues for increasing engagement. This presentation focuses on four key strategies that Cherokee has successfully used to engage providers: 1. organizational culture, 2. technology/data, 3. leadership and 4. the employee experience. Joel Hornberger, now Chief Strategy Officer, provides examples from his 25 years of work as COO for Cherokee Health Systems, a federally–qualified community health center (FQHC) and community mental health center in East Tennessee. As COO, he handled the day–to–day operations of its integrated care services, consisting of 46 medical, behavioral and dental clinics. |
Session B Motivational Interviewing to Engage Patients and Staff |
