Medicaid Accelerated eXchange (MAX) Series and MAX Training Program
The NYS Department of Health launched its approach to RCCI as the Medicaid Accelerated eXchange (MAX) program to support PPSs in their efforts to redesign the way healthcare is delivered. The key objective of the MAX program is to support the interdisciplinary PPS teams to accelerate their ability to achieve sustainable reductions in hospital admission and emergency department use. Through offering a series of RCCI workshops focused on improving care, the MAX program supports the goal of transforming the system by strengthening care collaborations and getting patients the right care at the right time. By understanding the drivers of high utilization, patients are directed to appropriate community providers and resources while promoting the more efficient use of hospitals for emergent and acute level services.

Transforming Care for Multi-Visit Patients Podcast
The Transforming Care for Multi-Visit Patients (MVP) podcast highlights lessons learned from the New York State Department of Health 2022 Medicaid Accelerated eXchange (MAX) Series program teams. The MAX Series, a delivery system transformation program sponsored by the New York State Department of Health (NYS DOH), was the fourth round of the program that originated from the NYS Delivery System Reform Incentive Payment (DSRIP) program. Each round of the MAX Series has successfully demonstrated that through a dedicated team-based approach it is possible to improve care for individuals whose underlying unmet needs result in high utilization of hospitals and emergency departments.
Dr. Amy Boutwell, developer of the MVP method and subject-matter expert, hosts conversations with the NYS DOH 2022 MAX Series lead and Chief Medical Officer for the Medicaid Program, Dr. Douglas Fish, and MAX Action Team members representing eight hospitals about their experiences through participation in the program and the impact their efforts made in the lives of the patients and families involved.
