Dear Ms. Kinslow-Evans:

We are pleased to inform you that the PPS of United Health Services Hospitals, Inc has been awarded an additional DSRIP Project Design Grant (PDG) Award.

The amount of your final PDG payment is $1.297 million. This award is intended to provide funding to support United Health Services Hospitals, Inc´s continued implementat ion planning and collaborat ion activities following the DSRIP application . We expect payment to be released in mid-February 2015.

In order to receive the PDG payment in a timely manner, you must complete the electronically enclosed Supplemental Award contract (Appendix X) within 5 business days. The executed contract must be mailed back to the following address:
