  • Increase the availability, accessibility and use of evidence-based interventions in self-care management in clinical and community settings
  • Increase the number of patients engaged in the asthma continuum of care
  • Increase utilization of asthma action plans and controller medication
  • Promote tobacco-free policies and smoking cessation in communities
  • Improve processes that support and increase engagement in prevention and self-management of diabetes and hypertension; provide educational services and resources for patients and providers; expand wellness initiatives; and expand opportunities for safe p hysical activity
  • Educate the public about the risks of opiate abuse
  • Increase primary care provider's knowledge of best practices and resources to address opiate use and addiction
  • Promote cross-system collaboration to optimize utilization and capacity of addiction services
  • Support collaboration of NYS Office of Mental Health, behavioral health providers and members of the tobacco control community to implement evidence-based strategies to reduce smoking among people with mental illness
