Technical Assistance and Training Resources for Prevention Agenda 2019-2024 Community Assessment, Planning and Implementation
The following sources provide technical assistance and training resources that will be useful to New York State local health departments, hospitals and community-based organizations as they implement Community Health Improvement Plans and Community Service Plans to achieve Prevention Agenda 2019-2024 goals and objectives.
- Finding Deep Leverage Points - A Starter Kit
- Selected "systems thinking" activities were developed by local and state partners as part of the "Connecting the Dots: Building Community Wealth for Food Security, Resilience and Well-Being" project with support from the 2020 County Health Rankings and Roadmaps Grant from the Robert Wood Johnson Foundation to Health Research Incorporated. These short activities can help public health practitioners begin identifying leverage points to build community wealth and food security.
- University at Albany School of Public Health's Center for Public Health and Continuing Education (CPHCE)
- The CPHCE site provides trainings, tools and resources for New York State local health departments, hospitals and community-based organizations who are working together to advance the Preventing Chronic Diseases Action Plan of the New York State Prevention Agenda through the implementation of their Community Health Improvement Plans and Community Service Plans.
- Translating Data Into Action
- The New York State Department of Health's Office of Public Health Practice convened an in-person meeting to bring together members of the Ad Hoc Committee to Lead the Prevention Agenda and representatives of local health departments, hospitals and other partner organizations across the state.
