Goals:
- Create community environments that promote and support healthy food and beverage choices and physical activity
- Prevent childhood obesity through early child-care and schools
- Expand the role of health care and health service providers and insurers in obesity prevention
- Expand the role of public and private employers in obesity prevention
- Achieve coordination of DSRIP efforts to promote tobacco use cessation, especially among low SES populations and those with poor mental health

- Apply the proposed the DOH DSRIP Project Plan for Tobacco Cessation
Interventions:
- CRH submitted and was selected for a mini-grant through the Mohawk Valley Coalition to adopt the 5-2-1-0 Let's Go Schoharie County program
- The $90k grant award will enable CRH to adequately invest in proven community-based programs that result in increased levels of physical activity and improved nutrition - 5-2-1-0 Everyday for schools, worksites, health care providers, and community, pres chool and after-school programs
- Increase collaboration with community partners (Creating Healthy Places, Complete Streets) and improve the retail availability of affordable healthy foods (making CRH a food-pick up point for Schoharie Fresh, the on-line Farmer's Market)
- Increase the number of schools and child care facilities that utilize the Let's Go 5-2-1-0 program to promote the availability of healthy meal and snack choices
- Incorporate time into the school day so that students have adequate time to engage in physical activity Introduce Bassett's Mileage Club to the Schoharie County Schools through collaborative efforts with BMC Mileage Club Sponsor
- Adopt healthy meal and beverage standards for meals sold and served at CRH
- Support school-based health centers in obesity prevention interventions 5-2-1-0; train staff in motivational interviewing techniques
- Conduct Lunch & Learn education sessions for community members on diet, exercise, stress, coping, obesity and disabilities
- Boost branding and awareness of 5-2-1-0 in Schoharie County Connect schools, CRH and the 5 Schoharie County Primary Care clinics to cross-promote obesity reduction activities, and nutrition via the proven messaging of 5-2-1-0
- Work collaboratively with Leatherstocking Collaborative Health Partners (LCHP) to address the general population with hot spot areas to include mental health practitioner offices, inpatient, outpatient, and emergency service facilities as well as communi ty, college, and business organizations
- Cessation Specialists available to inpatients
- Utilize Opt-to-Quit resources for staff training in the outpatient and Emergency setting
- Use the Butt-Stops-Here Program as the core curriculum for staff working directly with patients
- Through the DSRIP initiative, partnerships have been developed with the New York Tobacco Control Program, Seton Center for Smoking Cessation, the St
- Joseph Hospital Smoking Cessation Center, and other related health organizations
