How will the State assure that consumers not associated with TCM have a connection to a Health Home?

Consumers in need of care coordination services who qualify for Health Home services but are not currently engaged in care management will be among the first assigned to Health Homes.

Can we transition (and get paid for) 18 year olds in our TCM programs to our Adult Health Home?

Age does not preclude membership in a Health Home, however, children are not being prioritized for enrollment in the existing Health Homes while care models tailored specifically to children are being discussed.
