Admin | Will Request for Proposals (RFPs) be part of the expansion of HFW and more care management agencies? | An RFP is not required at this time. Reach out to OMH via OMH.HFW@OMH.NY.GOV to express interest in becoming a HFW care management agency. Visit the NYS System of Care Pilot webpage for more information. |
CANS | If a participant is referred for HFW and the CANS acuity does not come out high/complex, will this change the HFW the participant is receiving? | The child/youth must meet all eligibility requirements (which includes a rating of Complex on the CANS-NY) to enroll. However, the CANS-NY is not used to determine continued eligibility. Once a child/youth in enrolled in HFW, they remain enrolled regardless of the changing CANS-NY acuity rating. |
CANS | If participants acuity switched to complex after the recent CANS 2.1 adjustment, are they treated as having high acuity now? | Yes, their current acuity would be considered Complex based on the CANS 2.1 update. However, the HFW screening tool in the UAS must be completed for the participant to be considered for HFW. |
CANS | Can the Care Manager complete the Child and Adolescent Needs and Strengths (CANS) and HFW screening without the eligibility paperwork? | Supporting documentation and information gathering should occur prior and at the Children and Family Team Meeting (CFTM)/multidisciplinary team meeting to be able to accurately answer CANS tool items. Documentation supporting HFW eligibility must be on file prior to enrollment. |
Process | Is separate consent needed for entering data into the Uniform Assessment System (UAS) in addition to the Single Point of Access (SPOA) and the source? | Separate consent is not needed, as HFW is part of the Health Home Serving Children's (HHSC) Program |
Process | What is the anticipated workflow of enrollment for participants who are expected to transition home within 30 days? | The HFW Referral Process Webinar Scheduled for 3/20/25 will provide more detail. |
Process | Are the core services required to be in-person or can they be delivered virtually? | Please refer to the Core Services Requirements - High Fidelity Wraparound (HFW) Core Requirements within the HHSC Children's Program and the HFW policy Health Home HFW for Children/Youth with Serious Emotional Disturbance - Eligibility and Care Management Requirements. |
Process | Do requirements for HHSC and HFW such as assessments, HHSC Plan of Care (POC), and service planning need to be met? | Care Managers must adhere to all issued Health Home policies and procedures aside from Diligent Search Efforts. Diligent Search Efforts are not applicable to the HFW Program because that is not an expectation in HFW. |

Process | As a CMA not providing HFW, how do we apply to become a HFW CMA? | OMH is looking to expand the CMAs involved. There are 16 CMAs currently participating. In early summer 2025 OMH will open opportunities for additional CMAs to join. The Columbia Center for Practice Innovations is leading the training and coaching. Invites for trainings will be sent. Any questions should be submitted to OMH.HFW@OMH.NY.GOV |
Process | Can a participant with RE95 codes, who is not enrolled with Office of People with Developmental Disabilities (OPWDD), be accepted into HFW until they transfer to OPWDD? | Yes, if they meet the Health Home and HFW eligibility criteria. |
Process | How long does it take for a Care Manager to be trained for HFW? | The training takes a year (12 months) to complete. |
Process | Can a member transition from HFW to Care Management within the same CMA? | Yes, if the CMA has both the HH and HFW care management services. The HFW Referral Process Webinar scheduled for 3/20/25 will provide more detail. |
Process | What do the plans meetings look like if the family does not have support from the community? | Participant must be engaged in mental health services to be determined Serious Emotional Disturbance and meet eligibility for HFW. Participation and engagement of natural and community supports are required in HFW. Families are informed of this expectation and are supported to develop these relationships if they are not in place. |
Process | Does a HFW HH participant have a higher probability of gaining access to Children and Family Treatment and Support Services (CFTSS), HCBS, Residential Treatment Facility (RTF), and Mental Health/Behavioral specialist, compared to a non-HFW Health Homes member? | A child/youth enrolled in HFW should be prioritized to receive appropriate and chosen services to meet their needs and ameliorate the need for more intensive and/or out of home treatment and services. |
Process | Are there additional documents that need to be completed for enrollment/disenrollment from the HFW program? | The HFW Agreement Form does need to be completed upon enrollment. |
