Question 1: If a Health Home is an expert on TCGP VBP and is limited on counties, but the MCOs would like to work with you in other counties due to infrastructure, savings and performance demonstration. Can we work with them and how is state looking at these models?
Response: The DSRIP and Health Home Teams are working on this topic and guidance will be providing in the future.
Question 2: Where can we find information about the Health Home quality measures?
Response: The Health Homes Performance Management page has a comprehensive list of quality measures, related webinars, guidance, as well as information pertaining to CMART, MAPP Dashboards, and the Health Home Redesignation process.
Question 3: Is there a taxonomy code for Health Homes?
Response: There is no taxonomy code for Health Homes, but there is a category of service (COS) code of 0265 for Health Home/Care Management that is used for Medicaid billing purposes.
Question 4: Can a Health Home be required to get its own NPI?
Response: Prior to applying for COS 0265, an NPI is required. More information on how to apply for an NPI can be found on eMedNY´s website (Provider Enrollment Guide).
Question 5: Is registration for the upcoming Value Based Payment (VBP) Bootcamps available now?
Response: Registration for Value Based Payment Bootcamp in New York City on January 9, 2018 at the New York Academy of Medicine (1216 5th Ave, New York, NY 10029) will open on December 19, 2017 and will close on January 2, 2018. Registration for the upcoming VBP Bootcamp will be posted on the VBP Bootcamps webpage.
