Translating Data Into Action

The New York State Department of Health's Office of Public Health Practice convened an in-person meeting to bring together members of the Ad Hoc Committee to Lead the Prevention Agenda and representatives of local health departments, hospitals and other partner organizations across the state. More than 190 participants learned about local experiences in implementing Prevention Agenda activities and how health data from a variety of sources can guide local community health improvement efforts.

Meeting Objectives

  1. Identify and discuss interventions that advance health equity and reduce disparities.
  2. Explore how evidence-based implementation strategies are being adapted to address local challenges.
  3. Learn about tools for monitoring interventions and measuring health impact from Prevention Agenda website,
    County Health Rankings Action Center and other resources.
  4. Share ways to strengthen local partnerships and collaborations to improve community health.
