Post-Treatment Support Services for Breast Cancer Survivors who are Black

Overview

Post-Treatment Support Services for Breast Cancer Survivors is a New York State Department of Health program that provides support for breast cancer survivors who have either completed cancer treatment or will soon be completing treatment, with an added focus of supporting survivors who are Black to help address disparities in breast cancer outcomes. The Department currently funds grantees that serve nine counties throughout New York State. Grantees are community-based agencies that have experience providing breast cancer education, support, or wellness services and have breast cancer survivors in decision-making roles within the organization.

Program Activities

Grantees offer a range of free services to program participants. These include:

  1. Support groups that provide social and emotional support to participants as well as information to address the physical, mental, financial, and social impacts of cancer.
  2. One-to-one peer mentoring matching breast cancer survivors who are trained to assist other breast cancer survivors, especially those transitioning from active treatment to post-treatment. Mentors help address the emotional and psychological impact of breast cancer and the uncertainties of being a breast cancer survivor. They share knowledge, experience, and support.
  3. Referrals to cancer centers, primary care health providers, and community-based organizations to ensure breast cancer survivors in the post-treatment phase are provided with needed services in supportive environments.
