New York State Department of Health Cancer Community Programs List
- Cancer Services Program (CSP) - Breast, Cervical, and Colorectal Cancer Screening: CSP contractors offer free breast, cervical, and colorectal cancer screening and diagnostic services to eligible clients. Clients may receive clinical breast exams, mammograms, Pap tests and colorectal cancer screening through fecal tests. Clients receiving positive screening tests also receive diagnostic testing and are referred to treatment if needed. Eligible clients are also enrolled in the Medicaid Cancer Treatment Program for Medicaid coverage for the duration of their breast, cervical, colorectal or prostate cancer treatment.
- Community Outreach to Promote Colorectal Cancer Screening: These contractors conduct community outreach and provide education, linkage to community services, and referral to health care providers for colorectal cancer screening.
- Post-Treatment Support Services for Breast Cancer Survivors: These contractors provide support groups and one-to-one peer mentoring to breast cancer survivors, with an added focus of supporting survivors who are Black to help address disparities in breast cancer outcomes. Supportive services are offered to breast cancer survivors who have completed active treatment for their breast cancer, with a priority focus on those who are transitioning from active treatment to post-treatment care.
- Peer Education, Outreach, and Shared Decision Making for Persons at High Risk of Prostate Cancer: These contractors provide peer education, personalized coaching, linkage to community services to address barriers to health care, and referral to health care providers for prostate cancer screening with a focus on reaching Black individuals who bear a disproportionately high burden of prostate cancer.
