Centers of Excellence for Alzheimer's Disease
Ten Centers of Excellence for Alzheimer's Disease (CEADs) serve as the regional experts for the diagnosis and management of Alzheimer's Disease and related dementias (ADRD), provide the following services:
- integrative, comprehensive and coordinated medical services for the diagnosis of ADRD;
- management and treatment of patients with ADRD
- support and referral of patients, their caregivers and family members to community-based support services
- promote of the benefits of participation in research and referral to clinical trials;
- support for primary care and specialty care physicians caring for people with ADRD
- promote early detection and diagnosis by training of health care providers and students in health care professions on the diagnosis and treatment of ADRD;
- strong working relationships with community-based organizations and care providers
- promotion of public awareness about ADRD including the importance of wellness visits and cognitive screening

Alzheimer's Disease Community Assistance Program
Seven NYS Alzheimer's Association Chapters provide the following services to individuals with Alzheimer's disease and related dementias, their caregivers and family members, and members of the community:
- a 24-hour helpline offering information and referrals
- care planning and consultation
- education and training for caregivers
- support groups for people with ADRD, their caregivers and family members
- community awareness
- professional training

Alzheimer's Disease Regional Caregiver Support Initiative
Ten regional community-based organizations, in collaboration with strong community partners, provide the following support services to caregivers of individuals with Alzheimer's disease and related dementias to promote their mental and physical well-being:
- caregiver assessments
- outreach to and engagement with underserved communities
- caregiver support and engagement activities, such as:
- support groups;
- caregiver education;
- joint enrichment activities; and/or
- caregiver wellness programs
- respite*
* Respite services allow primary caregivers the opportunity to safely leave a person with ADRD for short or longer periods and can include overnight stays. Respite services are provided by volunteers or paid individuals. Respite services will be provided according to the level of patient function and caregiver need.
