Not all of the following signage may be required in all food service establishments, and additional signage may be required in some jurisdictions. Please contact your local health department for more information. Contact information can be found by using the Department's interactive map.
- The Food Allergen/Intolerance Notice available in Arabic, Bengali, Chinese, French, Haitian Creole, Italian, Korean, Polish, Russian, Spanish, Urdu, and Yiddish
- This statement: "If you have a food allergy or intolerance, please notify us." OR
- Allergen information for each prepared food item offered on the menu, such as "Contains peanuts" OR
- A statement that notifies customers that allergen information is available, such as "Allergen information for menu items is available. Ask an employee for details."

All menus, including those offered online, must include one of the following, in the same language as the menu:
Establishments with online menus for customers to place delivery or takeout orders must also provide a method for customers to inform the establishment if they have a food allergy or intolerance, unless the establishment lists allergen information for each menu item.
