RE: 2025 Home Care Cost Report Web-based Tool Launch
Dear Medicaid Provider Administrator:
Please be advised that the 2025 Home Care Cost Report is now available to submit your Agency’s Home Care Cost Report for CY 2025. Providers will have 90 days to complete the cost report. Cost report submissions will be due on Monday, August 31, 2026.
The Department of Health (DOH) and its contractor, KPMG, will be hosting a live 2025 Home Care Cost Report Kickoff Webinar tomorrow, June 11, 2026. You should have received the webinar registration information in your e-mail from us-advrisknyshc@kpmg.com. The live webinar will focus on changes, new updates and features of the 2025 cost report web-based Tool.
In addition to the Kickoff Webinar, more resources are available on the Department's website, including pre-recorded webinars about the Home Care Cost Report, instructions, background, key information, and Tool functionality. Please visit the Home Care Cost Report website for the Department's Home Care Cost Report materials.
Similar to previous years, providers can access the 2025 cost report by using the web-based Home Care Tool Dashboard. For users who have completed a Home Care Cost Report in a prior year, your login credentials for the web-based Tool will be the same. If your agency needs to add additional users to access the Tool, please send an inquiry to: us-advrisknyshc@kpmg.com, and please include the first name, last name, and email of the individual that needs access.
Please be reminded that Department will be unable to calculate a 2027 FFS reimbursement rates until the providers’ 2025 home care cost report(s) are submitted. In addition, failure to comply with the reporting requirements of Title 10, Parts 86-1.2, 1.3, 1.6 and 1.7 may result in the implementation of penalties pursuant to Part 86-1.2(c) and Section 12-d of the Public Health Law.
