March 28, 2025:
As part of ongoing efforts to monitor Managed Long Term Care (MLTC) plan oversight of Social Adult Day Care (SADC) sites, the New York State Department of Health (the Department) will be requiring MLTC plans to submit completed annual site evaluations for all currently contracted SADC sites which were completed August 2024 and after.
As a first step, your MLTC plan must provide a response to the below by April 11, 2025:
Please confirm whether your MLTC plan uses:
- The Suggested SADC Site Evaluation Tool for MLTC Plans that was issued by the Department in July 2024; or
- An alternative SADC site evaluation tool.
- If your MLTC plan uses an alternative site evaluation tool, please submit a blank copy of the tool in Excel format with your response to the Department.
- For example, if the site evaluation tool was completed in a format other than Excel, such as Microsoft Word, each field of the document must be transcribed into an Excel spreadsheet for submission to the Department.
Additional details regarding how and when to submit your plan’s completed annual SADC site evaluations will be forthcoming.

March 14, 2025: An updated SADC HCBS Compliance Roles & Responsibilities webpage is now live. The purpose of this webpage is to provide an overview of the HCBS compliance review process and the roles and responsibilities of the Department of Health, MLTC plans, and SADC sites within each phase of the review.
March 11, 2025: A new Guidance on Remediation Plans and Proof of Remediation webpage is now live. The purpose of this webpage is to provide guidance to MLTC plans and SADC sites on best practices for submitting acceptable remediation plans and proof of remediation as part of the HCBS compliance review process.
March 11, 2025: The HCBS Compliance FAQ webpage has been updated to include responses to questions received during the CY2025 SADC HCBS Compliance Process for MLTC Plans Webinar. The purpose of this webpage is to share responses to frequently asked questions received by the State Health Department regarding SADC compliance with the Centers for Medicare and Medicaid (CMS) Home and Community Based Services (HCBS) Settings Final Rule.
March 7, 2025: An updated SADC/SADS PCSP Template and a newly developed SADC/SADS PCSP Template User Guide are now live. The new template, or a SADC/SADS Template updated to match the Department template, should be utilized starting 3/7/2025 and going forward for all SADC/SADS PCSPs. The user guide is a resource to assist SADC programs with completing the SADC/SADS PCSP Template.
