DOH developed this website as a resource and repository for tools and information about compliance with the HCBS Final Rule. Please check back regularly to this announcement banner to learn about new or updated policies, email notifications, upcoming trainings or events, and other timely information including when new content is added to the website.


On 10/11/2024, DOH released a new version of the 'SADC/SADS Person Centered Service Plan (PCSP) Template' which is now available in the 'For SADC Sites' box below for immediate use.

The 'Personal Care Preference' section of the template has been expanded to include questions which specify whether the participant has a preference of who provides their care, whether the SADC can accommodate the preference and, if the SADC is unable to accommodate, if the member was notified and offered the option of switching to another SADC who can accommodate the preference.


On 10/11/2024, DOH released a new webpage, 'MLTC Plan SADC Contracting and Oversight Roles and Responsibilities' which is now available in the 'For MLTC Plans' box below.

The purpose of this page is to provide Managed Long Term Care (MLTC) plans with an overview of the roles and responsibilities for contracting with and providing oversight of Social Adult Day Care (SADC) centers in New York. This webpage covers the New York State Office of Aging (NYSOFA) regulations pertinent to the contracting and oversight of SADCs.


On 10/11/2024, DOH released a new webpage, 'SADC HCBS Community Integration' which is now available in the 'For MLTC Plans' and 'For SADC Sites' boxes below.

The purpose of this webpage is to provide Social Adult Day Care (SADC) providers with guidance related to community integration requirements. SADCs should ensure their policies and procedures are aligned with the guidance provided.
