Instructions for plan reporting on disparity-focused VBP arrangements (please include attachments as necessary with your response):
- Using regional data sources and claims data, identify 2 areas of demographic disparity in health outcomes for your membership.
- Provide detailed supporting analysis as to how you identified the disparities above. Please be sure not to include any protected information.
- Do you already have an arrangement that addresses these concerns? Y/N
- If Y, please include:
- Contract ID
- The timeline/start date of the arrangement
- Geographies included
- Member lives included
- Risk arrangement structure with contracted providers
- Which types of providers have entered this arrangement
- Summary of provider performance on quality improvement thus far
- If N, please provide plan of action to develop at least one targeted disparity-focused arrangement as described in the VBP Roadmap to be submitted to OHIP by January 1, 2027 and offered as a finalized arrangement for contracting no later than October 1, 2027. Please include details of how your plan will work with potential provider participants to collaborate on the details of the arrangement.
