How to Become an Approved New York State Early Intervention Service Provider

Apply Online to Become an Approved New York State Early Intervention Service Provider!

How to Apply:

  • Register as a new user in the Application System (create a username and password) here: https://pem.prod.nyeihub.com
  • Complete the Pre-Application Screening Tool (PAST) and submit.
  • The Pre-Application Screening Tool (PAST) will either send you a denial or a direct link to the full application, Provider Application Tool (PAT).
  • Complete the Provider Application Tool (PAT) and submit your online application.
  • Read all instructions very closely and monitor your email, the Bureau of Early Intervention Provider Approval Unit (BEI PAU) will contact you via email if additional information is required.

Minimum Requirements:

Individuals:

  • Applicants must be currently licensed and registered or certified in a profession recognized by the Early Intervention Program and qualified personnel in 69-4.1(al)(1-28) at Title: Section 69-4.1 - Definitions | New York Codes, Rules and Regulations (ny.gov)
  • Applicants must have documentation of a minimum of 1,000 clock hours prior to the date of application to the Department for approval, delivering such service to children under 5 years of age in an early intervention, clinical pediatric, early childhood education program, which may include relevant experience obtained as part of a supervised education program and/or clinical internship as a prerequisite for professional licensure, certification, or registration and provider that such experience must have included direct experience in delivering services to children with disabilities and their families.

Agencies:

  • Applicants must be a legal business entity authorized to do business in New York State which directly employs EI qualified personnel and may contract with individual providers which are approved by the Department, for the provision of early intervention program evaluations, service coordination and/or early intervention services.
  • Applicants must have an organizational structure that complies with all federal and state statutes and regulations, including practice acts established by the State Education Department Office of Professions.
  • Applicants must meet minimum agency staffing requirements in 69-4.5(a)(4)(vii) at Section 69-4.5 - Approval of service coordinators, evaluators and service providers | New York Codes, Rules and Regulations (ny.gov)

Types of Agreements:

Basic Agreement:

  • A Basic agreement is for individuals and agencies who will be contracting with an Early Intervention Program approved agency which has an Appendix 1 agreement with the Department.

Appendix 1 Agreement:

  • An Appendix 1 agreement is for individuals and agencies who will enroll in Medicaid to directly receive service authorizations from a Municipality and bill for Early Intervention Program services rendered.
  • Must have direct billing (private practice, private insurance, Medicaid, etc.) experience.

Central Directory of Early Intervention Services and Resources

1. The Central Directory* contains information concerning agencies and individuals with an approved agreement to provide evaluations, service coordination and early intervention services to children under the age of three with disabilities or delays and their families.

Approved Agencies/Municipalities can employ individuals who meet the qualifications identified in 10 NYRCC Section 69-4.1 or 4.4 (service coordinators) to deliver early intervention program (EIP) services. These additional agency/municipality employees who are rendering EIP services are not included in the Central Directory. Approved Agencies/Municipalities may contract with individual providers identified in the Central Directory who have an approved agreement to deliver EIP services.

The column titled "Service delivered in the last 12 months (Y/N)" indicates if the provider has actively delivered EIP services within 12 months of the posted date as identified by claim data.

The information contained in the Directory was last updated on March 19, 2026. The approval of additional providers and/or changes in personnel is ongoing therefore the Directory will be updated routinely. Current Providers with Appendix Agreements who would like to revise or add information recorded in the Central Directory (specifically: Mission, Experience, Service Coordination by County/Borough) can do so by contacting the Bureau's Provider Approval Unit (provider@health.ny.gov) to obtain a copy of the document titled Request to Revise Central Directory Information.

Your service coordinator or early intervention official can provide you with current information related to the early intervention services and supports offered in your county or region.

Recruitment and Retention of Qualified Personnel for Children with Disabilities

The National Center to Improve Recruitment and Retention of Qualified Personnel for Children with Disabilities (Personnel Improvement Center) is a federally funded technical assistance and dissemination project that was funded on October 1, 2008. Its mission is to increase the nation's capacity to recruit, prepare and retain diverse and highly qualified special educators, early intervention providers, and related service providers, including paraprofessionals, to meet the need.
