Social Care Networks (SCN)
Social Care Networks (SCNs) support delivery of health-related social needs (HRSNs) services for New York State (NYS) Medicaid members across NYS.
NYS established regional Social Care Networks (SCNs) to address health-related social needs (HRSNs) among NYS Medicaid members, aiming to improve health outcomes and reduce costs.

Goals of the SCN Program
- Expand access to HRSN services.
- Ensure consistent screening using the Accountable Health Communities (AHC) screening tool.
- Improve coordination across providers, community organizations and health plans.
- Coordinate screening and services using a close-loop referral system and Statewide Health Information Network.

SCN Program Authority
SCNs were established through the New York Health Equity Reform 1115 Waiver Demonstration Amendment, approved by the Centers for Medicare & Medicaid Services on January 9, 2024. This initiative funds screening, navigation, and Health Related Social Needs (HRS strengthening partnerships between health providers and community.

SCN Lead Entities:
- build networks of CBOs;
- coordinate screening and navigation services;
- support referrals and service delivery;
- enable secure data exchange;
- monitor program performance; and
- establish regional governance structures.
SCN Lead Entities also coordinate with:
- managed care plans;
- health care and behavioral health providers;
- local governments; and
- child and family service organizations.

How NYS Medicaid Members Can Get Connected
NYS Medicaid members can access SCN services by:
- going to the regional Social Care Network website and filling out the self-screener or calling their direct line
- speaking with their provider or care manager;
- contacting their health plan; or
- asking a community based organization that is participating in the network
