1115 Workforce Initiatives Service Commitment Site Directory
The Career Pathways Training (CPT) Program and Health Care Access Loan Repayment (HEALR) Program are two core Medicaid workforce initiatives authorized under New York's 1115 waiver that aim to address shortages in high-demand health care roles and boost the availability of critical staff in the health care workforce.
The table below lists the approved service commitment sites for the 1115 waiver workforce programs where participants in the CPT Program can fulfill their three-year service commitment and HEALR awardees can fulfill their four-year service commitment.
Program Applicants
If your current or prospective employer is not listed as an approved site below, please ask an authorized representative from the organization to complete and submit the Provider Attestation form (PDF) This form will be used to determine whether the location(s) where you currently work or plan to work meet the eligibility requirements to qualify as a service commitment site.
Employers
HEALR Program:
If you are an employer that would like to be considered for approval as a service commitment site, please have an authorized representative of your organization complete the Provider Attestation form (PDF) and submit it to healr@health.ny.gov for eligibility determination.
CPT Program:
If you are an employer that would like to be considered for approval as a service commitment site, please have an authorized representative of your organization complete the Provider Attestation form (PDF) and submit it to your Workforce Investment Organization (WIO) that serves your region. Please see CPT website for a list of counties served by each WIO.
To be an approved service commitment site, an organization, practice, or facility must meet one or more of the following criteria:
- The organization is an active Medicaid-enrolled provider with a patient population that was at least 30% Medicaid and/or uninsured members during one of the last three (3) years.
- The organization is a Social Care Network (SCN) lead entity
- The organization is a not-for-profit Community-Based Organization that is contracted with an SCN and is providing Health Related Social Needs (HRSN) screening, referrals, and/or services.
- The organization is a contracted not-for-profit provider partner of an SCN and is providing HRSN screening, referrals, and/or services (e.g. health or behavioral health provider).
*Article 16, Article 31 and Article 32 facilities may be eligible to serve as a service commitment site. These locations must meet the criteria indicated on the provider attestation form to qualify.
Note: Health systems must submit a separate Provider Attestation form, including the corresponding Medicaid ID number, for each of their locations. Approval of service commitment site eligibility is granted at the individual site/facility level.
