The designation process remains open and providers may continue to apply to be designated. The list will be continuously updated to reflect the addition of newly designated providers and for expansions in the array of approved services a provider may be designated to offer.

Sort by County instructions
To sort by a specific county or multiple counties, the Excel sheet should be filtered as follows:

  1. Select the filter for "County Served" and uncheck "Select All."
  2. Type the name of the county you are searching. For example: "Albany." Click "OK" or hit Enter. All provider sites serving Albany County will be displayed.
  3. If you would like to add an additional county, for example if you want to see all provider sites serving Albany and Schenectady counties, select the filter for "County Served" again and type the next county ("Schenectady") and check the box to "Add current selection to filter" and click "OK". Do NOT simply click the names of the counties in the filter's drop down box, as this will ONLY display selections where the county checked is the only county listed.
  4. Repeat for as many counties as you would like to include in your search.

The links below offer policies and guidance related to the provision of Children's HCBS. Please note, this is not an exhaustive list, and providers are encouraged to explore the 1915c website in its entirety to discover all policies related to Children's HCBS. If you're a newly designated Children's HCBS provider, refer to the New York State Children's Waiver Designated Home and Community Based Services (HCBS) Provider Attestation for required action needed before providing services. Children's HCBS Providers must establish an HCS account to enable them to complete background checks and access the Incident Reporting And Management System (IRAMS), etc. The Department will help to establish HCS accounts for providers following HCBS designation, including the IRAMS linkage referenced below. Children's HCBS Providers are subject to specific background checks, as outlined in policy CW0001. The Department will help to facilitate onboarding and account creation to comply with these requirements following HCBS designation. Please reach out to bh.transition@health.ny.gov for assistance.

Children's HCBS Providers are required to submit reportable incidents, complaints, and grievances through the Incident Reporting and Management System (IRAMS), as outlined in the policies below. This requires access to HCS and IRAMS. The Department will help to facilitate onboarding and account creation to comply with these requirements following HCBS designation. Children's HCBS providers are required to report on the capacity of HCBS by county, service and waitlist in the Children's Services Capacity Tracker, which is located within the Incident Reporting and Management System (IRAMS), which is housed in a secure platform of the Health Commerce System (HCS). The State is available to provide technical assistance to new and existing HCBS providers at any time. Newly designated HCBS providers who have been designated through the Non-licensed/Designated/Certified Designation Policy should expect to have a check-in meeting with the State around 6 months from the date of their Formal Designation Meeting.

The New York State (NYS) Department of Health (DOH), Office of Addiction Services and Supports (OASAS), Office of Children and Family Services (OCFS), Office of Mental Health (OMH), and the Office for People with Developmental Disabilities (OPWDD) collaborate to review providers interested in applying for Children's HCBS and/ or CFTSS.

Providers must be designated through this process to provide Children's CFTSS and/or HCBS services under the NYS Medicaid program (both fee-for-service Medicaid and Medicaid Managed Care):

Children and Family Treatment and Support Services (CFTSS) include Other Licensed Practitioner, Crisis Intervention, Community Psychiatric Supports and Treatment, Psychosocial Rehabilitation, Family Peer Support Services, Youth Peer Support.

Children's Home and Community Based Services (HCBS) include Caregiver /Family Advocacy and Support Services, Crisis Respite, Planned Respite, Supported Employment, Community Habilitation, Palliative Care: Counseling and Support Services, Palliative Care: Pain and Symptom Management, Palliative Care: Expressive Therapy (art, music, play), Palliative Care: Massage Therapy, Prevocational Services, and Day Habilitation.

What You Need Before Being Granted Access to the Application:

This application requires providers to have:

  • A user ID (granted by the NYS OMH)
  • A Facility Code
