The purpose of this survey is to identify barriers to accessing Children's HCBS and report issues to the State in real time. HHCM/C-YES complete for all Children's HCBS referrals, one per provider only when referrals are not successful.

Children's Services Capacity Tracker
Children's HCBS providers are required to report on the capacity of HCBS by county, service and waitlist in the Children's Services Capacity Tracker, which is located within the Incident Reporting and Management System (IRAMS), which is housed in a secure platform of the Health Commerce System (HCS). Access and connectivity will be facilitated by the Department as part of HCS and IRAMS onboarding. Please reach out to bh.transition@health.ny.gov for assistance.
