Please make sure to fill in the form completely; including the title of the material(s) wanted, the pub number(s) of the material, language of the material, the quantities of each material, as well as appropriate contact and address information. Once the form is filled in completely, please email it to OGS.SM.GDC@ogs.ny.gov. Once an order form is submitted, please allow 4-6 weeks for delivery.
